Clinical Characteristics according to the presence of lung parenchymal tuberculosis in patients with tuberculous pleurisy

Abstract
Purpose: To find associated factors with the development of tuberculous pleurisy, We compared the clinical data according to the primary or reactivated tuberculous pleurisy which were defined by the radiological findings.

Materials and Methods: In all patients >18 years old who were diagnosed as tuberculous pleurisy at Chungbuk National University Hospital from 2010 to 2011, we defined the activity of pulmonary tuberculosis and presence of reactivation by analyzing lung parenchymal lesion. Thereafter we compared the characteristics of radiological findings, pleural fluid data, underlying disease and clinical data in each group.

Results: In thirty seven patients with tuberculous pleurisy, twenty seven patients(73%) had lung parenchymal or lymph nodal tuberculosis lesion regardless of activity. Sixteen patients(43%) had active pulmonary tuberculosis. Eighteen patients(49%) had reactivation pattern and nineteen(51%) had primary tuberculosis pattern. The patients with reactivation pattern were more older than the patients with primary tuberculosis pattern(64.6 and 41.5 years, p< .002), had more frequent comorbidity(50% and 5 %, p<0.003) and lower level of peripheral blood hemoglobin(11.75 g/dL and 13.40 g/dL, p<0.014).

Conclusion: The patients of tuberculous pleurisy by reactivation of tuberculosis were more older, had more frequent comorbidity associated with immunity and more lower peripheral blood hemoglobin level than primary tuberculous pleurisy.
